ABSTRACT:
Effective discharges begin with admissions. By implementing technology such as a discharge referral system, hospitals and post-acute care facilities can manage the patient flow process at a holistic, comprehensive level, enabling two key efficiencies:

  • Bridge the disparate islands of excellence that exist within the healthcare system, improving coordination across the hospital, and with post-hospital care providers
  • Connect the many moving parts of the healthcare delivery process and better manage the complex patient care coordination and discharge planning processes

The time for healthcare organizations to act is now to prevent further delay in discharge planning. By 2030, the U.S. population age 65 and older is projected to exceed 65 million. This increase will undoubtedly strain an already fragile health system. It is imperative that the industry prepare to meet the needs of future patients.
